Are You Ready For A Lifestyle Change?

Directions: Indicate "yes" or "no" to each of the following questions by placing a check mark in the appropriate column. If you can answer "yes" to the following questions, you are ready to begin a lifestyle change program.

  1. Do you view lifestyle change as a lifetime goal rather than a temporary, short term goal?
