Advance Directives/Organ Donation

Choose one of the following two activities:

1. Visit the site for Choice in Dying and download a copy of the advance directive that is appropriate for your state. Briefly describe the form: What type of document is it? What situations does it cover? What provisions does it make? Finally, describe whether reviewing the document has made you more or less likely to complete an advance directive.
