Present Situation

Now John must use a wheelchair for the rest of his life. He could not return to the group home because it did not have the facilities necessary to accommodate his wheelchair. His parents have been struggling to find another group home for John. Due to federal and state cutbacks in mental health, there are few facilities or living options available to the chronically mentally ill. There are even fewer options available to the chronically mentally ill with special needs. At this point John is living in a state mental hospital, which is not an appropriate placement for him. When John is on medication he is able to function at a fairly high level. When he stops his medication, his hallucinations and delusions return, requiring intervention.

The following conditions are found to be correlated to successful treatment outcome with people who have schizophrenia. Using a 1 to 10 rating scale, rate where you believe John would fall on the scale. Then, in a short sentence, present the evidence to justify your rating.

1 - Poor
5 - Neutral
10 - Excellent

1. Premorbid adjustment: The higher the level of functioning before the onset of the disorder, the better the prognosis for recovery.

2. Precipitating event: If there is an identifiable event that seemed to trigger the schizophrenic response, the better the chance for a favorable recovery.

3. Onset: The more sudden or acute the onset of the schizophrenic event, the better the prognosis.

4. Age of onset: The later in life the initial schizophrenic onset occurs, the better the prognosis.

5. Affect: The presence of emotions, including depression and anxiety, is a positive sign. Hopelessness without depression appears to be a poor sign.

6. Type of schizophrenia: Undifferentiated, chronic schizophrenia has the poorest prognosis. Paranoid schizophrenia with acute onset has the best prognosis for recovery.

7. Support group and family: Engaged and concerned friends and family who are educated about schizophrenia assist in providing more positive outcomes.

8. Response to treatment: Positive prognosis is linked to positive responses to medications and psychotherapy.

After finishing your rankings and writing the justifications for them, write a brief statement of what you believe John's prognosis is, given the limited information in the case study.
